We believe support should meet people where they are. LifeLink offers 1915(i) home and community-based services as a Medicaid-contracted provider through Alliance Health. We have particular focus and experience supporting youth transitioning out of foster care — helping them build the daily living skills, community connections, and stability they need as they move toward independent adulthood. Our Community Living & Support, Individual & Transitional Support, and Supported Employment services also support individuals with intellectual or developmental disabilitiesand those navigating behavioral health needs. Wherever someone is on that path, our team brings hands-on support, patience, and a familiar face — so independence feels achievable, not overwhelming.

Yes! All of our staff are fully trained and background-checked, with ongoing training specific to 1915(i) services — including community living support, transitional skills coaching, and supported employment — so they're equipped to meet each individual's unique needs.
